How to Get PCA services in MN

If your child has access to the Medicaid program, and utilizes straight MA, your child may qualify for Personal Care Attendant services.

To qualify for PCA services, your child must show Level 1 behaviors. Level 1 behaviors must be documented by a professional. A doctor, psychologist or psychiatrist are qualified professionals that may document your child's behaviors.

  1. Call MN Visiting Nurse Agency at 612-617-4600 and ask for an assessment for your child. You do NOT need a referral from another agency to call.
  2. Make sure that you have a psychologist, psychiatrist or medical doctor that is willing to document your child’s behaviors. This is a primary requirement.
  3. Fill out the level 1 behavior form, thinking about your child’s worst day and continuing issues.
  4. The agency will set up a time (about 2 hours) to visit with you and fill out the assessment. The child must be present at this appointment. Only one guardian is necessary to be present.
  5. Turn in your paperwork promptly, including the signed doctor behavior form.
  6. You will need to re-qualify for these services each year.
